Bariatric Revision Surgery Options after Band, Sleeve & Gastric Bypass

Bariatric surgery is one of the most effective tools for long-term weight loss and improvement of obesity-related health conditions. In fact, many patients are satisfied with the outcomes of their initial weight loss surgery (WLS). However, weight and metabolism are influenced by many factors over time (hormones change, life circumstances evolve, and obesity is a chronic disease), so some patients may benefit from additional support years after their original surgery.

What Is Bariatric Revision Surgery? 

A bariatric revision surgery, or just "revision” as it’s commonly called, is simply any WLS that modifies or converts a previous weight loss procedure. Most often, patients come to Dr. Cribbins at Texas Center for Bariatrics & Advanced Surgery for a revision for the following reasons:  

  1. Weight loss has slowed or plateaued earlier than expected 

  1. Regained some or all of their lost weight over the years 

  1. Experiencing persistent symptoms or medical concerns (like acid reflux)  

Needing additional treatment does not mean the original surgery didn’t work. Or that the patient didn’t “do the work.” Obesity is a long-term medical condition, and sometimes treatment needs to be adjusted, just like any other chronic condition.

Revision Options by Original Procedure 

After the Lap-Band Procedure 

The Lap-Band was a widely used surgical device for many years and, while it is still sometimes the preferred patient option, some eventually chose to remove the devise due to discomfort, limited weight loss, or lifestyle challenges. Because the Lap-Band is a device positioned around the stomach, it can be removed so the patient can receive a conversion to gastric sleevegastric bypass, or SADI-S

Like many of our patients, Anika had a revision from Lap-Band to SADI-S in 2024 and within 9 months lost 116 lbs. She says:

After a Gastric Sleeve Procedure 

The sleeve is an excellent option for many patients, but some may experience changes over time that bring them to Texas Center for Bariatric & Advanced Surgery to discuss revision options or medical weight management.  

Surgical revision options include a conversion to gastric bypass (most common), or a conversion to SADI-S. Gastric Bypass may be recommended for patients with significant or persistent reflux or moderate weight regain, while the SADI-S may be considered when greater metabolic support is needed, weight regain is substantial, diabetes or metabolic disease remains difficult to control.

After a Gastric Bypass Procedure 

A gastric bypass provides durable results for many patients, but the body’s anatomy and metabolism can change over time. Years after surgery, it’s possible for the pouch or connection to stretch, for metabolic adaptation to occur, or for weight regain and symptoms to develop. Contrary to popular belief, there still are revision options if the original surgery was a gastric bypass (RNY). Revision options may include: 

1. Pouch or outlet revision 

2. Limb length adjustment to enhance metabolic effect 

3. Conversion to SADI-S in carefully selected patients. 

Carly received a revision with the help of Dr.Cribbins and has already lost 75 pounds. She says:

After SADI-S 

SADI-S is a powerful metabolic procedure, and most patients achieve strong, lasting results. In some cases, after a SADI-S procedure, adjustments may be needed to support continued healthy weight loss or address new symptoms that develop over time. When appropriate, revision options may include: 

  1. Limb length modification to optimize absorption 

  1. Nutritional management or correction 

  1. Structural revision if necessary
